Precision Medicine - Realizing the Whole Patient Story

The difference between a traditional assessment of a complex patient condition and true “precision medicine” is the degree of reliance on available data – at point of care – to make decisions about specific treatment paths that may be more beneficial for the patient.

To achieve a path to true personalized healthcare we need to step back from locking data in disparate silos (genomic, EMR, claims, etc.) and start thinking of building a patient model that reflects the whole patient story with all these pieces fitting together and allowing patterns to emerge from this picture.
